States and Territories: Please note that a website has been created to serve as a resource for applicants to the National Policy Summit to Address Behavioral Health Disparities within Health Care Reform. Please visithttp://conference.afyainc.com/emhdpolicysummit/index.html
This website contains general information about the Policy Summit and the application process. It will be expanded to include a Frequently Asked Questions (FAQ) section responding to general inquiries, as well as a reference section offering the opportunity to view materials developed during the 2009 Policy Summit. Logistics and Resource sections will later become available to selected delegations in preparation for their participation.
Additionally, the 2011 National Policy Summit website provides interested parties with the convenience of an online application. If you prefer, you may also email the documents to firstname.lastname@example.org. Please ensure that Tribal representatives are included as appropriate, and note that the inclusion of key constituencies is a critical part of the review process.
Recognizing the time commitment that engaging multiple constituencies in the completion of a comprehensive application requires, the deadline for submission has been extended to March 1, 2011.
